Can't use modal in forms

I can’t use modal in starter form with this code:

But i can’t use modals in forms
what should i change to implement this logic in my other forms too?
should i add any camunda native endpoint ?
(esentially, when i try to use pop-up modals i am readressed to unknownd camunda page)

Could you share the complete code snippet? It looks like a part of it is missing.
