Form failure: The context path is either empty or not defined.
I already packed it in the .war and pasted it back on the camunda server

How have you deployed your process?
For me, it seems you have used the Camunda Modeler to deploy your process .
Please place your .bpmn in ProcessApplication and try to re-deploy it again. That should solve your problem
